Give your form a Title — this is required. A short Description underneath is optional.
Click + Add a field once for every question you want on the form.
For each field: type the question (e.g. "Employee Name"), then choose what kind of answer it needs — Short answer, Date, Paragraph, Dropdown, Checkbox, or Centered Title (for section headings like "Approvals").
Check Required if that question must be answered. Check Same row as field above to place two fields side by side, like Name and Date on one line.
Use the ↑ / ↓ buttons to reorder a field, or ✕ to remove it.
Watch the Live preview on the right — it updates as you go, so you always see exactly what the form will look like.
When it's ready, click Save Form. It appears in "Open a form" right away, so you can open it and try it out immediately.
A couple of things to know
Nothing you type is ever saved to a server — it lives in your browser and disappears if you close the tab.
A form you create is available on this device only, right away, for testing. To make it available to everyone else, it needs to be deployed — check with whoever manages the site.
